IMPORTANT REMINDER:

If your student plans on participating in the upcoming Fall Season, please visit our website to complete the athletic contract and submit an updated Physical Exam (attached)    NO LATER THAN August 13

ImPACT Testing:  Any new student-athletes who wish to participate in fall sports, or existing student-athletes who have not had ImPACT testing done in the past two years will also need to attend one of the baseline ImPACT testing dates below (testing needs to be re-done every 2 yrs and takes approximately 30 min's):

• Tuesday, August 7th /  3pm - 5pm
• Wednesday, August 8th / 3pm - 5pm

Physical Exams:  For the 2018-2019 sports season, all athletes must turn in a current  physical exam performed on or after June 1, 2018 prior to their respective sport season (fall, winter, spring).  Exam paperwork should be turned in as soon as possible by mail, fax or drop-off in the main office.

Yearly physical exams are best done by your primary care provider due to their knowledge of your health history and vaccination record.  However, if your physical examination is dated prior to June 1, 2018, the clinics listed below offer reduce cost examinations in the summer to verify ability to participate in strenuous athletic activities throughout the school year.

Access Sports Medicine & Orthopaedics offers examinations on the following dates at the specified location from 5pm - 7pm for $5.  No appointment is necessary.  If you have questions, contact Sports Medicine at 775-7575.

• Tuesday, August 7 - Raymond
• Thursday, August 9 - Exeter

* Wear a t-shirt and shorts to your examination and bring a completed health history (front page of the physical form- attached).  Athletes under 18 years old need to be accompanied by a parent/guardian.
